Recovery does not end when you leave our facility. We view recovery as an ongoing journey, and our role is to be the starting gate, providing the foundation you need to build a life that lasts. Aftercare planning begins early in your stay, not the week before discharge.

Why Aftercare Matters

Most relapses happen within the first 90 days after leaving treatment. The transition from structured inpatient care back to daily life is one of the most vulnerable periods in recovery.

That is why we start planning for life after OAR Wellness from week one, not as an afterthought, but as a core part of your treatment.

Our aftercare approach includes:

  • Written discharge and relapse-prevention planning
  • Identification of high-risk situations and personal triggers
  • Connection to community support and medical support resources
  • Clear pathway back to more intensive care if needed

Discharge Planning Starts Early

Discharge planning isn't something we rush through in your final week. It's integrated into your treatment from the beginning.

This plan is written, reviewed with you, and updated before you leave. You will not be discharged without a clear roadmap for the months ahead.

Relapse Prevention Planning

Relapse isn't failure, it's a signal that something needs attention. But prevention starts with awareness and preparation.

During treatment, we help you:

  • Identify your personal triggers and high-risk situations
  • Build coping strategies using skills learned in therapy
  • Create a concrete action plan for moments of craving or crisis
  • Know when and how to reach out for support
  • Recognize warning signs before a full relapse occurs

Your relapse-prevention plan is tailored to your life, triggers, and support system. We treat this as a practical tool you can use.

Community Support Connections

Recovery is more sustainable with the community. Isolation is one of the biggest risk factors for relapse. 

Before you leave, we help connect you with:

  • Local recovery groups (including SMART Recovery and 12-step options)
  • Online recovery groups
  • Outpatient therapists and counsellors
  • Physicians for medical follow-up
  • Peer support networks in your area

We do not prescribe one path. Some people thrive in 12-step programs. Others prefer SMART Recovery or individual therapy. We help you find what works for you.

Alumni Support & Ongoing Connection

Leaving inpatient treatment does not mean losing connection with OAR Wellness.

We offer alumni support to help you stay connected:

  • Alumni groups for ongoing peer connection
  • Check-ins with our team as needed
  • Access to resources and referrals as your needs evolve
  • Clear pathway back to inpatient care if required

Family Aftercare Support

Recovery affects the whole family, and family support does not end at discharge. 

We offer ongoing resources for families, including:

  • Continued family education and communication coaching
  • Guidance on supporting recovery at home
  • Resources for managing challenges that arise after treatment
  • Connection to family alumni groups

Family participates weekly during treatment, and we help them prepare for their ongoing role in your recovery journey.

Recovery isn't linear. There will be difficult days, and sometimes those days turn into difficult weeks.

Continued therapy and support are available for the client as desired. Additionally, family aftercare support is available for up to 30 days post-program completion.
