We retain your personal health information for the period required by law and professional standards. In Ontario, health records must generally be retained for a minimum of 10 years from the date of the last entry, or 10 years after the client reaches 18 years of age (whichever is later).

●    Access your information: Request a copy of your personal health information in our records
●    Correct your information: Request corrections to inaccurate or incomplete information
●    Withdraw consent: Withdraw consent for certain uses or disclosures (subject to legal limitations)
●    File a complaint: If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer or with the Information and Privacy Commissioner of Ontario
